This policy explains what information we collect when you use the Executive Healthspan Score screening tool, how we use it, and your rights.

Who we are

This screening tool is operated by Healthspan by Design, the executive health optimization practice of Andres Preschel, MD. References to "we," "us," and "our" refer to Healthspan by Design.

What we collect

Information you give us

• Contact details: first name, last name, email address, phone number, and (optionally) LinkedIn profile URL when you submit the lead form to view your results.
• Screening responses: your answers to the 12 questions in the Executive Healthspan Score, including biological sex, blood pressure (if provided), and self-reported lifestyle and family history information.

Information collected automatically

• Technical data: IP address, browser type and version, device type, screen size, operating system, referring URL, pages and screens viewed, time spent, and timestamps.
• Advertising attribution: click identifiers and campaign parameters (such as utm_source, utm_campaign, fbclid) passed in the URL when you arrive at the site, used to measure ad performance.
• Browser storage: we use your browser's session storage to remember your attribution parameters during your visit. Session storage is automatically cleared when you close the tab. We do not use long-term tracking cookies on this page beyond what is set by Meta Pixel (see below).

How we use your information

• To generate and deliver your personalized Executive Healthspan Score and report.
• To contact you by email or phone about your results, our Executive Optimization Programs, or scheduling a consultation.
• To measure and improve the screening tool and our advertising campaigns.
• To comply with legal obligations and protect against fraud or misuse.

Who we share information with

We do not sell your personal information. We share it only with the service providers and partners we use to operate the screening and follow up with you:

• Zapier — receives your form submission and routes it to our internal systems.
• Monday.com — our customer relationship system, where your contact details and responses are stored so our team can follow up.
• Calendly — the scheduling tool we use if you book a consultation. Calendly's own privacy policy applies once you click the booking link.
• Meta (Facebook/Instagram) — we use the Meta Pixel on this site to measure the effectiveness of our advertising and to build audiences. The Pixel may share information about your visit with Meta, including your IP address, the actions you take, and pseudonymous identifiers used for ad matching.
• Hosting and infrastructure providers — companies that host the website and deliver the underlying technology.
• Legal disclosures — if required by law, court order, or to protect our rights, safety, or that of others.

Your choices and rights

Depending on where you live, you may have the following rights regarding your personal information:

• Access — request a copy of the information we hold about you.
• Correction — ask us to correct information that is inaccurate.
• Deletion — ask us to delete your information, subject to legal retention requirements.
• Opt out of marketing — ask us to stop emailing or calling you at any time.
• Non-discrimination — we will not deny services or charge you differently for exercising these rights.

Advertising and Meta Pixel

We advertise on Meta platforms (Facebook and Instagram). The Meta Pixel records when you visit this page, when you start the screening, complete it, submit your details, and click the consultation booking link. This helps us measure ad performance and show relevant ads to people likely to benefit from our services.

You can manage Meta's use of your information through your Facebook Ad Preferences or your Instagram privacy settings. You can also opt out of interest-based advertising through industry tools at aboutads.info and networkadvertising.org.

Important: this is not medical care

The Executive Healthspan Score is an educational screening tool. It is not a medical diagnosis, not a substitute for evaluation by a licensed physician, and is not subject to HIPAA in the same way a clinical encounter would be. Your responses are treated as marketing and contact information, not as a protected medical record. If you become a patient of Healthspan by Design through a separate consultation and engagement, the privacy practices of that medical relationship will be provided to you separately at that time.

Data retention and security

We keep your information for as long as we need it to deliver the screening, contact you about our services, and meet our legal and accounting obligations. We use industry-standard protections such as HTTPS encryption in transit and access controls on our internal systems. No internet transmission or storage is perfectly secure; we cannot guarantee absolute security.

Children

This screening is for adults. We do not knowingly collect information from anyone under the age of 18. If you believe a minor has submitted information, please contact us so we can delete it.

California residents (CCPA/CPRA)

If you are a California resident, you have additional rights under the California Consumer Privacy Act, including the right to know, the right to delete, the right to correct, the right to opt out of the "sale" or "sharing" of personal information, and the right to limit the use of sensitive personal information. We do not sell personal information for money. We may "share" personal information for cross-context behavioral advertising as defined under California law (for example, through the Meta Pixel). To opt out, contact us using the details below or use the global privacy control browser signal where available.

Changes to this policy

We may update this policy from time to time. When we do, we will update the "Last updated" date at the top of this page. Material changes will be communicated through the website.
